FREQUENTLY ASKED QUESTIONS

WHAT IS THE PROCESS TO START HOME HEALTH SERVICES?

Depending on your insurance, services can begin in many different ways.  Most of our patients are referred to us through their Case Managers.  If you have a case manager, inform them that you would like State Health Care Ltd to be your home health agency.  Your case manager will then contact us with a referral and insurance authorization to begin your services.  Our nurse would then send out an order to your doctor to approve your services.  Then our nurse would contact you to discuss your individual needs and set up a time to come out and do an assessment in order to begin your services.  

If you don't have a case manager and you have Medicaid, you would fill out a referral form, which would give our DON all the information she needs to contact your doctor to send out an order for your services.  She would then call you to set up a time to come out and do your assessment.  After the assessment and the return of the order from your doctor, your services would begin.

ARE YOUR PRACTITIONERS CERTIFIED PROFESSIONALS?

Yes!  Home Health Aides are required to complete certified training before they are allowed to work.  All of our aides have been trained and certified.  HHAs are also required to complete 12 hours a year of continuing education, as well as be certified in CPR and Basic First Aide.

Our RNs and LPNs are also state certified and required to have continuing education in order to retain their licenses.  Nursing licenses are checked each year.  Nurses are also required to be certified in CPR and Basic First Aid.

WHAT INSURANCES DO YOU ACCEPT?

We accept Buckeye, Molina, Medicaid, Passport, ComCare, Aetna Better Health of Ohio, Ohio Home Care Waiver, and DODD/SELF/Level 1/IO Waiver.
